Narrative:

How problem arose: inadequate taxiway markings on airport surface and on commercial airport diagrams. We were cleared to spot #6, then told to turn left which placed us on taxiway a. We were at intersection of taxiway D and taxiway west and sequenced to follow a company aircraft in front of us. The controller asked us to cross runway 27R at taxiway D. I had the 11-0 chart which does not show taxiway D, but the taxiway D sign was right there at the intersection. The first officer had the 10-9 and misread the taxiway D. How discovered: controller told us while we were on runway 27R that he wanted us on taxiway D. We asked what he wanted us to do now. He said go ahead across and hope we have room for you on other side. We crossed on taxiway west and were able to pull up far enough to clear runway 27R. We apologized but indicated the taxiway markings were not very good. He agreed and said no problem. Contributing factors: familiarity sometimes is our own worst enemy. Pilots and controllers need to be aware of any changes to the routine. We should have been more vigilant, the controller could have been more specific, i.e., use taxiway D, cross runway 27R right on 'aa.' this would have forced us to consult diagram to see logical route to 'aa.' WX was good and the departure bank had just started. Sometimes to rush too much when the WX is good, maybe we should slow the pace just a little when we have lots of runway xings on an active runway. The best intentions of a good crew were negated by not asking for help from a busy controller.

HOW DISCOVERED: CTLR TOLD US WHILE WE WERE ON RWY 27R THAT HE WANTED US ON TXWY D. WE ASKED WHAT HE WANTED US TO DO NOW. HE SAID GO AHEAD ACROSS AND HOPE WE HAVE ROOM FOR YOU ON OTHER SIDE. WE CROSSED ON TXWY W AND WERE ABLE TO PULL UP FAR ENOUGH TO CLR RWY 27R. WE APOLOGIZED BUT INDICATED THE TXWY MARKINGS WERE NOT VERY GOOD. HE AGREED AND SAID NO PROB. CONTRIBUTING FACTORS: FAMILIARITY SOMETIMES IS OUR OWN WORST ENEMY. PLTS AND CTLRS NEED TO BE AWARE OF ANY CHANGES TO THE ROUTINE. WE SHOULD HAVE BEEN MORE VIGILANT, THE CTLR COULD HAVE BEEN MORE SPECIFIC, I.E., USE TXWY D, CROSS RWY 27R RIGHT ON 'AA.' THIS WOULD HAVE FORCED US TO CONSULT DIAGRAM TO SEE LOGICAL RTE TO 'AA.' WX WAS GOOD AND THE DEP BANK HAD JUST STARTED. SOMETIMES TO RUSH TOO MUCH WHEN THE WX IS GOOD, MAYBE WE SHOULD SLOW THE PACE JUST A LITTLE WHEN WE HAVE LOTS OF RWY XINGS ON AN ACTIVE RWY. THE BEST INTENTIONS OF A GOOD CREW WERE NEGATED BY NOT ASKING FOR HELP FROM A BUSY CTLR.
