2 separate events: small aircraft takeoff from palm beach county park (lna). Pbi approach contacted at 1000 ft MSL for transition northbound at 4500 ft MSL. Squawk code issued. Apparent missed radio call to proceed wward. Instructed to expedite right turn to heading 180 degrees. 5 SM north pbi, under radar vectoring at 2000 ft MSL, twin engine passed off left wing at 200 ft, same altitude, head-on (sbound). ATC controller asked for twin engine altitude verification of 2000 ft MSL. ATC controller then asked for altitude verification from me of 2000 ft MSL at time of near miss. ATC sounded unsure of aircraft separation.
