Narrative:

First domestic trip in 19 months. First trip sequence in aircraft type which holds joint rating with widebody transport type. Fourth day of trip sequence operating late due to maintenance problems, primarily APU inoperative. Arrival at previous stations, mechanics did not know about problem until notified. Mco also had to be notified of need for conditioned air and external air for starting. Also new domestic procedure for flight plans and WX. Called up by ticket agent at passenger check-in position at gate. However, printer did not work after several attempts and switched to another position to obtain paperwork. Back on aircraft, off line pilot requesting jump seat. Briefed on oxygen, etc. Crew meals arrive, stow in cockpit. I entered the route to destination in the FMC with a mccoy 7 departure as per the flight plan. Mechanic comes on board, informs me access door to external air start panel is missing. Exit aircraft to inspect and write up. Door obviously not latched properly previous station and torn off in flight. After write-up and new MEL, established procedure with maintenance for start and after start procedure. It required opening to be covered with speed tape and #2 engine to be crossbled and started. Reviewed additional special procedure requiring slight weight reduction. Conditioning air disconnected and jetway moved away. External air unit would not bring N3 to start minimum. Mechanic advised at least 15 mins for new unit. Elderly passenger began to complain of heat. Several mins spent attempting to get maintenance to reconnect conditioned air, which was finally accomplished. No cooling air, however. The jetway was brought back and the temperature probe was put on board the aircraft. Finally, cool air. New air unit brought up and conditioned air and jetway removed. Start now successful. Pushback after panel was taped and #2 engine started via crossbleed. Checklist and taxi. No weight and balance through data link. Takeoff numbers received over radio. First officer manually computes numbers for balanced thrust takeoff. Accomplished before takeoff check. First officer made takeoff on runway 18L with radar vectors to a slow gradual right turn to the northeast to intercept mccoy 7 departure. At that time I made an entry into the FMC which skipped the slight dog leg on the departure to crg VOR and initiated a direct course to sav VOR which was the second fix on the departure. First officer intercepted the selected course. Center soon advised we were right of course and gave us a vector to the left (northwest) to reintercept the departure. At that time, the SID plate was checked and the proper leg to crg was entered and intercepted. The various interruptions and non-routine evens on the ground were distrs which caused deviations from normal operating procedures. Although checklists were used, the takeoff briefing did not include naming the first fix after takeoff. In flight an improper entry to the FMC was not immediately recognized because 1 pilot was not utilizing VOR data. The correct procedures are in place but must be adhered to 100 percent. When the backup safeguard is not utilized 1 time, that is when a deviation may occur.

HOWEVER, PRINTER DID NOT WORK AFTER SEVERAL ATTEMPTS AND SWITCHED TO ANOTHER POS TO OBTAIN PAPERWORK. BACK ON ACFT, OFF LINE PLT REQUESTING JUMP SEAT. BRIEFED ON OXYGEN, ETC. CREW MEALS ARRIVE, STOW IN COCKPIT. I ENTERED THE RTE TO DEST IN THE FMC WITH A MCCOY 7 DEP AS PER THE FLT PLAN. MECH COMES ON BOARD, INFORMS ME ACCESS DOOR TO EXTERNAL AIR START PANEL IS MISSING. EXIT ACFT TO INSPECT AND WRITE UP. DOOR OBVIOUSLY NOT LATCHED PROPERLY PREVIOUS STATION AND TORN OFF IN FLT. AFTER WRITE-UP AND NEW MEL, ESTABLISHED PROC WITH MAINT FOR START AND AFTER START PROC. IT REQUIRED OPENING TO BE COVERED WITH SPD TAPE AND #2 ENG TO BE CROSSBLED AND STARTED. REVIEWED ADDITIONAL SPECIAL PROC REQUIRING SLIGHT WT REDUCTION. CONDITIONING AIR DISCONNECTED AND JETWAY MOVED AWAY. EXTERNAL AIR UNIT WOULD NOT BRING N3 TO START MINIMUM. MECH ADVISED AT LEAST 15 MINS FOR NEW UNIT. ELDERLY PAX BEGAN TO COMPLAIN OF HEAT. PUSHBACK AFTER PANEL WAS TAPED AND #2 ENG STARTED VIA CROSSBLEED. CHKLIST AND TAXI. NO WT AND BAL THROUGH DATA LINK. TKOF NUMBERS RECEIVED OVER RADIO. ALTHOUGH CHKLISTS WERE USED, THE TKOF BRIEFING DID NOT INCLUDE NAMING THE FIRST FIX AFTER TKOF. IN FLT AN IMPROPER ENTRY TO THE FMC WAS NOT IMMEDIATELY RECOGNIZED BECAUSE 1 PLT WAS NOT UTILIZING VOR DATA. THE CORRECT PROCS ARE IN PLACE BUT MUST BE ADHERED TO 100 PERCENT. WHEN THE BACKUP SAFEGUARD IS NOT UTILIZED 1 TIME, THAT IS WHEN A DEV MAY OCCUR.
