Narrative:

Flight from slc to dfw received ACARS clearance which read slc slc 094 degrees. 2 lines later in the next paragraph it stated the slc 4 departure. The first officer was flying and never read the slc 4 departure so, upon reaching the slc VOR, he began a right turn to join the 094 degree radial (takeoff from runway 34L). Shortly thereafter, departure control called to ask heading. Then they stated we should be on runway heading as stated on the slc 4 departure. Had we not been climbing that quickly, we could have hit the mountains east of slc. The captain did not question the first officer's turn and I do not have charts to know what is on sids and stars. Also, I did not read clearance beyond the transponder code. From now on, I will try to have a better understanding of clearance to the extent possible. The largest problem here was the captain not paying full attention to the operation of his aircraft. He obviously did not read the SID. I plan on talking to our professional standards committee after the month is over.

Title: FO OF ACR LGT ACFT FAILED TO FOLLOW THE ASSIGNED SID AFTER TKOF RESULTING IN ATC INTERVENTION.

Narrative: FLT FROM SLC TO DFW RECEIVED ACARS CLRNC WHICH READ SLC SLC 094 DEGS. 2 LINES LATER IN THE NEXT PARAGRAPH IT STATED THE SLC 4 DEP. THE FO WAS FLYING AND NEVER READ THE SLC 4 DEP SO, UPON REACHING THE SLC VOR, HE BEGAN A R TURN TO JOIN THE 094 DEG RADIAL (TKOF FROM RWY 34L). SHORTLY THEREAFTER, DEP CTL CALLED TO ASK HDG. THEN THEY STATED WE SHOULD BE ON RWY HDG AS STATED ON THE SLC 4 DEP. HAD WE NOT BEEN CLBING THAT QUICKLY, WE COULD HAVE HIT THE MOUNTAINS E OF SLC. THE CAPT DID NOT QUESTION THE FO'S TURN AND I DO NOT HAVE CHARTS TO KNOW WHAT IS ON SIDS AND STARS. ALSO, I DID NOT READ CLRNC BEYOND THE XPONDER CODE. FROM NOW ON, I WILL TRY TO HAVE A BETTER UNDERSTANDING OF CLRNC TO THE EXTENT POSSIBLE. THE LARGEST PROB HERE WAS THE CAPT NOT PAYING FULL ATTN TO THE OP OF HIS ACFT. HE OBVIOUSLY DID NOT READ THE SID. I PLAN ON TALKING TO OUR PROFESSIONAL STANDARDS COMMITTEE AFTER THE MONTH IS OVER.
