Narrative:

Doing simultaneous ILS runways 27, I was low side runway 27R, turned air carrier X in and called traffic to him, air carrier Y for parallel runway. I believe air carrier X said he saw air carrier Y so I didn't use other separation means but went about doing other things. That was a problem. Air carrier Y got a TCASII advisory. That started the ball rolling, tapes being pulled and so on. Came back to work next day to find out I had a 'deal' previously.

Narrative: DOING SIMULTANEOUS ILS RWYS 27, I WAS LOW SIDE RWY 27R, TURNED ACR X IN AND CALLED TFC TO HIM, ACR Y FOR PARALLEL RWY. I BELIEVE ACR X SAID HE SAW ACR Y SO I DIDN'T USE OTHER SEPARATION MEANS BUT WENT ABOUT DOING OTHER THINGS. THAT WAS A PROB. ACR Y GOT A TCASII ADVISORY. THAT STARTED THE BALL ROLLING, TAPES BEING PULLED AND SO ON. CAME BACK TO WORK NEXT DAY TO FIND OUT I HAD A 'DEAL' PREVIOUSLY.
