Narrative:

Departed runway 9 released from south bend -- wind 090 at 12 KTS, light rain, VFR! Aircraft was shooting VOR 27 at beh and said she gave an advisory 1/2 mi out. Active runway was 9. After liftoff in climb both aircraft on converging head on course took evasive action to avoid collision. South bend did not have traffic on screen. Aircraft was at MDA below radar coverage, but was not using south bend approach for radar advisories. Had she been talking with approach, I would have never been given a release without an advisory. Simple use of advisory service while practicing approachs would have kept this from happening. Please call if more information is necessary. Callback conversation with reporter revealed the following information: reporter stated that the other aircraft did not use safe good procedures in making an approach to the non controled airport on a time that the WX wind favored the opposite end of the runway to which the approach was being conducted without receiving TA's from approach control. The other pilot may have announced her intentions on the airport unicom frequencys, however, it was not heard by the reporter even though he monitored the airport unicom taxiing out for takeoff. He further stated that both aircraft took evasive action by making a turn to their right.

Title: PLT OF CPR SMT ACFT HAD AN NMAC WITH ANOTHER ACFT APCHING HEAD-ON TO THE OPPOSITE END OF THE RWY DURING CLBOUT.

Narrative: DEPARTED RWY 9 RELEASED FROM S BEND -- WIND 090 AT 12 KTS, LIGHT RAIN, VFR! ACFT WAS SHOOTING VOR 27 AT BEH AND SAID SHE GAVE AN ADVISORY 1/2 MI OUT. ACTIVE RWY WAS 9. AFTER LIFTOFF IN CLB BOTH ACFT ON CONVERGING HEAD ON COURSE TOOK EVASIVE ACTION TO AVOID COLLISION. S BEND DID NOT HAVE TFC ON SCREEN.
