Narrative:

When preflting aircraft, the pitot cover for the coplts instruments was not removed. This was not discovered until into the takeoff roll. Due to confusion when it was noticed that the airspds did not match, V2 was passed and a takeoff was made. As soon as we were airborne, I realized what had occurred and tool over control of the aircraft from the copilot who had made the takeoff. I asked for a return to the airport and made a normal landing. The cover was removed and the rest of the flight proceeded normally. The basic problem was complacency -- doing the preflight just like a hundred times before and not really paying attention to what I was doing.

Title: AN LTT JET FREIGHTER CREW FAILED TO REMOVE A PITOT COVER DURING THEIR PREFLT INSPECTION.

Narrative: WHEN PREFLTING ACFT, THE PITOT COVER FOR THE COPLTS INSTS WAS NOT REMOVED. THIS WAS NOT DISCOVERED UNTIL INTO THE TKOF ROLL. DUE TO CONFUSION WHEN IT WAS NOTICED THAT THE AIRSPDS DID NOT MATCH, V2 WAS PASSED AND A TKOF WAS MADE. AS SOON AS WE WERE AIRBORNE, I REALIZED WHAT HAD OCCURRED AND TOOL OVER CTL OF THE ACFT FROM THE COPLT WHO HAD MADE THE TKOF. I ASKED FOR A RETURN TO THE ARPT AND MADE A NORMAL LNDG. THE COVER WAS REMOVED AND THE REST OF THE FLT PROCEEDED NORMALLY. THE BASIC PROBLEM WAS COMPLACENCY -- DOING THE PREFLT JUST LIKE A HUNDRED TIMES BEFORE AND NOT REALLY PAYING ATTN TO WHAT I WAS DOING.
