Narrative:

Air carrier maintenance program requires propeller overhaul/change at 5 yrs or 3000 hours, whichever first occurs. 5 yr due date was 9/92. Operator's maintenance planning personnel missed this date. Likewise, 9 flcs assigned to the aircraft missed this date. Discrepancy was discovered on 10/92. Propeller time was 2197.2 hours -- under the 3000 hour tbo but beyond the 5 yr due date. Aircraft was grounded, pending accomplishment of propeller change/overhaul. Additionally, operator conducted a fleet-wide campaign to assure that similar discrepancies had not occurred on other aircraft. Operator is revising its aircraft status report forms and computer program implementing a mandatory periodic review of aircraft status to assure that incidents of this nature do not recur. Supplemental information from acn 223232. Before every flight, the PIC is required to ensure that the aircraft is airworthy. Pilots are furnished an aircraft status sheet listing the due dates of all airworthiness inspections and components and systems. All due dates are listed in the left column except for one item, the propeller change/overhaul, which is listed in the lower right hand corner. This overhaul/change date was inadvertently overlooked by both flight operations and maintenance personnel. The aircraft was flown past that date. Supplemental information from acn 223439. I was assigned a flight from fresno, ca, to oakland, ca, to carry freight on a scheduled run. I did a preflight on the aircraft which included looking over all the paperwork and aircraft logs for the aircraft to check and make sure the aircraft was in airworthy condition. I overlooked the propeller time limitation. The chief pilot alerted me to this problem and it has since been corrected. Supplemental information from acn 223524. As a pilot for aviation corp the PIC is to insure that the aircraft is airworthy. Each day the PIC checks the 'aircraft status report' see insert. On 10/92 I flew an aircraft, with the propeller out of tbo. A total of 7 pilots flew that same airplane. The tbo on the propeller is 3000 hours or 9/92. After going back and looking at the status report for that airplane the status report was hand written, like this insert, the date on aircraft was 09-97 or 92. The total time on aircraft is approximately 1440 hours. Insert: this 'aircraft status report' is currently a copy out of the airplane I am flying now. Please note the hand-written hours for the propeller. Is it 3000 hours or 3500 hours? These 'aircraft status reports' should be typed or ran through a computer with a printer.
