Narrative:
I was working aircraft X and Y at 9000. I was distracted when new crew came on duty. Looked away from radar. When I saw my aircraft, they were approximately 4 mi and converging. I turned both. They saw each other at about 2 mi. I reported this to my supervisor who filed an error package.

Title: ACFT X HAD LTSS FROM ACFT Y. SYS ERROR.
Narrative: I WAS WORKING ACFT X AND Y AT 9000. I WAS DISTRACTED WHEN NEW CREW CAME ON DUTY. LOOKED AWAY FROM RADAR. WHEN I SAW MY ACFT, THEY WERE APPROX 4 MI AND CONVERGING. I TURNED BOTH. THEY SAW EACH OTHER AT ABOUT 2 MI. I RPTED THIS TO MY SUPVR WHO FILED AN ERROR PACKAGE.
