Narrative:

This is a follow up for your information, report filed on this incident 12/X/91. Large transport came out of 'C' check locking pin not removed from right ground spoilers. Ground spoilers locked in full extended position (should have had red flag attached to pin, none attached). Not detected on preflight as company procedure is to deploy all spoilers prior to outside walk around, so all spoilers were up during flight engineer check, which is normal. There is no requirement to check spoiler retraction once speed brake handle returned to stowed position. So! We took off with spoiler up. It caused roll and yaw plus caused #3 engine to violently compressor stall continuously until shut down. Roll problem quite severe. 5 units rudder 50 degree left aileron -- to fly straight -- very little ability to turn left. (Thrust from #1 plus spoiler). Landed uneventfully. Fortunately I am an large transport check captain and we experimented around with this stuff in the simulator all the time, so it wasn't a big deal, but coupled with bad WX or mexico city, etc. It could have been a disaster. Supplemental information from acn 195898. After takeoff (captain flying) aircraft flight controls had a problem and #3 engine started compressor stalling. A precautionary shutdown was accomplished and fuel was dumped. Flight controls still had a major problem. An emergency was declared and fire trucks were standing by for us. We found our inboard right ground spoilers were fully deployed. At this time maintenance thinks that a lock out was forgotten by maintenance after a heavy 'C' check, and the disrupted air after takeoff flamed-out our #3 engine, and caused the obvious flight control problems.

Narrative: THIS IS A FOLLOW UP FOR YOUR INFO, RPT FILED ON THIS INCIDENT 12/X/91. LGT CAME OUT OF 'C' CHK LOCKING PIN NOT REMOVED FROM R GND SPOILERS. GND SPOILERS LOCKED IN FULL EXTENDED POS (SHOULD HAVE HAD RED FLAG ATTACHED TO PIN, NONE ATTACHED). NOT DETECTED ON PREFLT AS COMPANY PROC IS TO DEPLOY ALL SPOILERS PRIOR TO OUTSIDE WALK AROUND, SO ALL SPOILERS WERE UP DURING FLT ENGINEER CHK, WHICH IS NORMAL. AT THIS TIME MAINT THINKS THAT A LOCK OUT WAS FORGOTTEN BY MAINT AFTER A HVY 'C' CHK, AND THE DISRUPTED AIR AFTER TKOF FLAMED-OUT OUR #3 ENG, AND CAUSED THE OBVIOUS FLT CTL PROBLEMS.
