Narrative:

Taxi out for takeoff on 1 engine. Upon arrival at end of runway we were cleared to takeoff ahead of other aircraft, but completed startup of other engine in sufficient time to complete checklist in demand-response mode. As I pushed throttles up, we got takeoff warning horn, exited runway to discover flaps incorrectly set. PNF had read checklist and I responded, but he omitted the flap item and I didn't notice it as he read checklist. Even reading checklist can produce undesired results, if there are not subjective backups.
