Narrative:

Small aircraft received IFR clearance from newport news ground and was subsequently taxied to runway 7 for departure. The local controller was confused about the type of flight plan the small aircraft was on. Thinking that the small aircraft was a VFR departure the aircraft was cleared for takeoff and switched to orf departure at 6 mi. This constituted a controller deviation in that the IFR aircraft entered the airspace of another controller without prior coordination. The controller in charge was notified of the incident on oct/mon/91 at XX30 hours local. Contributing factors: ground, clearance and flight data were combined under a heavy workload. No strip was passed to local. The pilot didn't state he was 'IFR, ready for departure.' I believe that should be mandatory for all private aircraft. Local did not hear ground state 'small aircraft is IFR.' corrective actions: mandate non commercial aircraft state IFR or VFR when calling for takeoff clearance. Make strips for all departing aircraft. Better coordination procedure.

Title: IFR SMA CLRED FOR TKOF AND ENTERS DEP CTL AIRSPACE WITHOUT A RELEASE OR HDOF. POOR INTRAFAC INTERFAC COORD.

Narrative: SMA RECEIVED IFR CLRNC FROM NEWPORT NEWS GND AND WAS SUBSEQUENTLY TAXIED TO RWY 7 FOR DEP. THE LCL CTLR WAS CONFUSED ABOUT THE TYPE OF FLT PLAN THE SMA WAS ON. THINKING THAT THE SMA WAS A VFR DEP THE ACFT WAS CLRED FOR TKOF AND SWITCHED TO ORF DEP AT 6 MI. THIS CONSTITUTED A CTLR DEV IN THAT THE IFR ACFT ENTERED THE AIRSPACE OF ANOTHER CTLR WITHOUT PRIOR COORD. THE CTLR IN CHARGE WAS NOTIFIED OF THE INCIDENT ON OCT/MON/91 AT XX30 HRS LCL. CONTRIBUTING FACTORS: GND, CLRNC AND FLT DATA WERE COMBINED UNDER A HVY WORKLOAD. NO STRIP WAS PASSED TO LCL. THE PLT DIDN'T STATE HE WAS 'IFR, READY FOR DEP.' I BELIEVE THAT SHOULD BE MANDATORY FOR ALL PRIVATE ACFT. LCL DID NOT HEAR GND STATE 'SMA IS IFR.' CORRECTIVE ACTIONS: MANDATE NON COMMERCIAL ACFT STATE IFR OR VFR WHEN CALLING FOR TKOF CLRNC. MAKE STRIPS FOR ALL DEPARTING ACFT. BETTER COORD PROC.
