Narrative:

I was trained to depart jets off runway 18 with an immediate left turn so as to simulate a runway 13R departure without coordinating with dfw regional approach. Our LOA does not address jet departures off runway 18. This practice has been taken place with management's approval. On oct/fri/91 management put a briefing statement in binder #1 that we should attempt to obtain a release from dfw approach prior to departing jets off runway 18. On my first day back to work on oct/sat/91 we were on a north flow and I did not read or initial off on the new change. When I signed on oct/sun/91 I was still not familiar with the new change. Later on that sunday afternoon I cleared cpr Y light transport for takeoff on runway 18 with an immediate left turn heading 130. I was working local control east, west, radar 1, and coordinator position combined at the time of the occurrence. I was not briefed of the new changes to jet departures off runway 18 before I took the position. The dfw supervisor called out controller in charge to report a system deviation on the jet departure off runway 18. I was decertified pending the outcome of management investigation on monday. I then read the #1 binder and I was still not sure if it was a mandatory requirement or it was a request to alert dfw regional approach that a jet would be departing runway 18. I had been absent from work the past 3 weeks prior to that sunday for the exception of 3 days the previous week on sick leave.

ON MY FIRST DAY BACK TO WORK ON OCT/SAT/91 WE WERE ON A N FLOW AND I DID NOT READ OR INITIAL OFF ON THE NEW CHANGE. WHEN I SIGNED ON OCT/SUN/91 I WAS STILL NOT FAMILIAR WITH THE NEW CHANGE. I WAS NOT BRIEFED OF THE NEW CHANGES TO JET DEPS OFF RWY 18 BEFORE I TOOK THE POS. THE DFW SUPVR CALLED OUT CIC TO RPT A SYS DEV ON THE JET DEP OFF RWY 18. I WAS DECERTIFIED PENDING THE OUTCOME OF MGMNT INVESTIGATION ON MONDAY.
