Narrative:

Cleared visibility approach behind widebody transport jet, 1 O'clock from our position. First officer flying aircraft. We both looked to 1 O'clock and picked up what we thought to be widebody transport at 5 mi. Also 1 O'clock on TCAS. As we turned in on base about 45 seconds later to follow, I looked back to ensure our belly was clear and saw another aircraft, co-altitude and about 3 mi on collision course. As we started evasive action (climb), approach called to climb immediately to 4000', so we did. TCAS did give RA during climb. We passed directly over widebody transport. Controller had lost bubble earlier. I guess when he called the 1 O'clock traffic he meant the 12:30 traffic, which was not on TCAS in 10 mi scale. All we had on both TCAS and visibility was (we found later) an large transport.

Title: FLT CREW OF MLG CLEARED FOR VISUAL TO FOLLOW TRAFFIC MISIDENTIFIED TRAFFIC AND HAD TRAFFIC CONFLICT WITH CLOSER TRAFFIC.

Narrative: CLRED VIS APCH BEHIND WDB JET, 1 O'CLOCK FROM OUR POS. F/O FLYING ACFT. WE BOTH LOOKED TO 1 O'CLOCK AND PICKED UP WHAT WE THOUGHT TO BE WDB AT 5 MI. ALSO 1 O'CLOCK ON TCAS. AS WE TURNED IN ON BASE ABOUT 45 SECS LATER TO FOLLOW, I LOOKED BACK TO ENSURE OUR BELLY WAS CLR AND SAW ANOTHER ACFT, CO-ALT AND ABOUT 3 MI ON COLLISION COURSE. AS WE STARTED EVASIVE ACTION (CLB), APCH CALLED TO CLB IMMEDIATELY TO 4000', SO WE DID. TCAS DID GIVE RA DURING CLB. WE PASSED DIRECTLY OVER WDB. CTLR HAD LOST BUBBLE EARLIER. I GUESS WHEN HE CALLED THE 1 O'CLOCK TFC HE MEANT THE 12:30 TFC, WHICH WAS NOT ON TCAS IN 10 MI SCALE. ALL WE HAD ON BOTH TCAS AND VIS WAS (WE FOUND LATER) AN LGT.
