Narrative:

During final approach on 2R at bna, the final controller cleared us for approach but not over to tower. Captain was running radios and both of us failed to switch to tower. Three at fault. Captain, copilot, and controller who forgot about us. We were at the end of another 13-14 hour duty day and that always contributes. Prevention by sticking to landing regimen, PF keeping eye on PNF duties. These 14 hour days and scheduled reduced rest will cause something more serious to happen.

Narrative: DURING FINAL APCH ON 2R AT BNA, THE FINAL CTLR CLRED US FOR APCH BUT NOT OVER TO TWR. CAPT WAS RUNNING RADIOS AND BOTH OF US FAILED TO SWITCH TO TWR. THREE AT FAULT. CAPT, COPLT, AND CTLR WHO FORGOT ABOUT US. WE WERE AT THE END OF ANOTHER 13-14 HR DUTY DAY AND THAT ALWAYS CONTRIBUTES. PREVENTION BY STICKING TO LNDG REGIMEN, PF KEEPING EYE ON PNF DUTIES. THESE 14 HR DAYS AND SCHEDULED REDUCED REST WILL CAUSE SOMETHING MORE SERIOUS TO HAPPEN.
