Narrative:
While I was working radar position I noticed a potential conflict about to occur between 2 aircraft at the same altitude. I immediately turned both aircraft and issued traffic. The small transport X reported the other aircraft Y in sight at which time I told him to maintain visibility sep.

Title: APCH CTLR TURNED TWO ACFT TO MAINTAIN SEPARATION, BUT WAS TOO LATE AND LESS THAN STANDARD SEPARATION OCCURRED.
Narrative: WHILE I WAS WORKING RADAR POS I NOTICED A POTENTIAL CONFLICT ABOUT TO OCCUR BTWN 2 ACFT AT THE SAME ALT. I IMMEDIATELY TURNED BOTH ACFT AND ISSUED TFC. THE SMT X RPTED THE OTHER ACFT Y IN SIGHT AT WHICH TIME I TOLD HIM TO MAINTAIN VIS SEP.
