Narrative:

After takeoff from sfo runway 01R (440000# takeoff gross weight) using normal takeoff thrust (as opposed to reduced thrust), we missed the altitude restriction of 3000' at the sfo 6 DME by approximately 500'. We used our company's standard climb procedure. Both pilots reviewed the text of the SFO4.sfo SID (effective 10/90). The restriction is not mentioned in the text following takeoff runways 1L/right; it is only shown in the graphic. We both read the text but did not find the restriction on the familiar but recently revised graphic. This climb restriction should be mentioned in the text (as similar restrictions are at all other airports). This omission is potentially dangerous and should be corrected. Supplemental information from acn 160823: the plate is not explicit enough on this restriction, as the minimum climb gradient narrative does not predominately note the hard altitude restriction.
