Narrative:

Our flight from fra to dtw was on a random track across the north atlantic. At lakes, montreal center reclred us from over acker via nar 328 to dtw. The original flight plan was filled lakes dct acker dct yfm J379 NM J567 Y16 J551 yyb J551 eck dct. As the instrument pilot in the right seat I was giving IOE to a new captain and we had within the previous hour briefed on nattracks and north american rtes. The IOE captain had been quizzed on several ways to apply common/non common nars from coastal fixes to destination. Nar 328 goes from lakes dct acker dct chiboo (mt) J545 yee, Y20, V4C and spica dtw. The IOE captain set up the new flight plan positions and I read the coordinates to him from the canadian high chart 3. The problem was recognized over chiboo as the INS was programmed to go dct yee, montreal center requested we go pct val dor (yvo) and upon setting the course or 253 degrees into yvo we were about 1 1/2 dots east of centerline J545 on our HSI. Center made a comment about flying the non common portion with the common portion which was what we thought we were doing, but J545 goes midland and beyond, but the initial dogleg out of chiboo is to val D'or which VOR is not stated on the non portion. The previous 5 hours of this flight were predicated on INS navigation solely, and though the remainder would be done on INS the HSI switches were in the rad position. Having instructed over this route before to and from all of our european destinations, I should have caught the missed waypoint in the INS loading, but due to the instructional material being covered and the lack of more complete annotation on the non common portion of this route, we were slightly off the centerline of the arwy out of chiboo when center called. About 1/2 way between chiboo and val D'or center cleared us dct midland. This flight was scheduled for 9+15 hours and I had been instructing continually about INS fixes, aireps and service engine drift down procedure prior to receiving the new clearance. There was a little fatigue factor on both our parts and this was his first atlantic trip. To preclude this overnight in the future, I suggest a chart printout for the VOR idents on non common portions and certainly the first VOR fix on the arwy after coming off a pure INS environment.

THE IOE CAPT HAD BEEN QUIZZED ON SEVERAL WAYS TO APPLY COMMON/NON COMMON NARS FROM COASTAL FIXES TO DEST. NAR 328 GOES FROM LAKES DCT ACKER DCT CHIBOO (MT) J545 YEE, Y20, V4C AND SPICA DTW. THE IOE CAPT SET UP THE NEW FLT PLAN POSITIONS AND I READ THE COORDINATES TO HIM FROM THE CANADIAN HIGH CHART 3. THE PROB WAS RECOGNIZED OVER CHIBOO AS THE INS WAS PROGRAMMED TO GO DCT YEE, MONTREAL CENTER REQUESTED WE GO PCT VAL DOR (YVO) AND UPON SETTING THE COURSE OR 253 DEGS INTO YVO WE WERE ABOUT 1 1/2 DOTS E OF CENTERLINE J545 ON OUR HSI. CENTER MADE A COMMENT ABOUT FLYING THE NON COMMON PORTION WITH THE COMMON PORTION WHICH WAS WHAT WE THOUGHT WE WERE DOING, BUT J545 GOES MIDLAND AND BEYOND, BUT THE INITIAL DOGLEG OUT OF CHIBOO IS TO VAL D'OR WHICH VOR IS NOT STATED ON THE NON PORTION. THE PREVIOUS 5 HRS OF THIS FLT WERE PREDICATED ON INS NAV SOLELY, AND THOUGH THE REMAINDER WOULD BE DONE ON INS THE HSI SWITCHES WERE IN THE RAD POS. HAVING INSTRUCTED OVER THIS RTE BEFORE TO AND FROM ALL OF OUR EUROPEAN DESTS, I SHOULD HAVE CAUGHT THE MISSED WAYPOINT IN THE INS LOADING, BUT DUE TO THE INSTRUCTIONAL MATERIAL BEING COVERED AND THE LACK OF MORE COMPLETE ANNOTATION ON THE NON COMMON PORTION OF THIS RTE, WE WERE SLIGHTLY OFF THE CENTERLINE OF THE ARWY OUT OF CHIBOO WHEN CENTER CALLED. ABOUT 1/2 WAY BTWN CHIBOO AND VAL D'OR CENTER CLRED US DCT MIDLAND. THIS FLT WAS SCHEDULED FOR 9+15 HRS AND I HAD BEEN INSTRUCTING CONTINUALLY ABOUT INS FIXES, AIREPS AND SVC ENG DRIFT DOWN PROC PRIOR TO RECEIVING THE NEW CLRNC. THERE WAS A LITTLE FATIGUE FACTOR ON BOTH OUR PARTS AND THIS WAS HIS FIRST ATLANTIC TRIP. TO PRECLUDE THIS OVERNIGHT IN THE FUTURE, I SUGGEST A CHART PRINTOUT FOR THE VOR IDENTS ON NON COMMON PORTIONS AND CERTAINLY THE FIRST VOR FIX ON THE ARWY AFTER COMING OFF A PURE INS ENVIRONMENT.
