Narrative:

Climbing through 12;000 feet and 250 KIAS; a moderate to severe vibration was felt throughout the airframe. Returned to field for immediate landing. Post flight inspection revealed left inboard thrust reverser clamshells had up to 2 inches of separation from engine nacelle. It appears that maintenance had been performed the previous evening indicated by a blue security seal on the main cabin door. During cockpit preparation check; the windshield heat switches were in the off position; which were turned on for normal ops. There was a [pilot write-up] maintenance form in the aircraft logbook under the discrepancy tab which had a diagonal line drawn through sections 1; 2 & 3. Section 4 was completed with mechanic's signature. The [pilot write-up] which meled the thrust reversers had been moved in the logbook from the MEL discrepancy tab to the aircraft discrepancy tab. In discussing the issue with the maintenance controller on duty; he related that an incorrect part had been delivered to repair the thrust reverser discrepancy. It was discovered through the work process that the incorrect part was on hand and that things were put back together in order to keep the jet in operation. I suspect that the reassembly was not done correctly after discovering that the incorrect part had been discovered. Unsure if thrust reversers can partially deploy if the thrust reversers are electrically and mechanically disabled if the MEL procedure is properly complied with.
