Narrative:

Upon arrival at aircraft for departure, aircraft log book was found with a sheet of paper taped to the cover, with the correct flight # hand written on it. All crew members checked the log book, but failed to verify the correct log with aircraft registration #. 'Wrong' log was used. Stronger vigilance is needed by all personnel in contact with log book.

Title: ACR LGT DISPATCHED WITH WRONG LOG BOOK.

Narrative: UPON ARR AT ACFT FOR DEP, ACFT LOG BOOK WAS FOUND WITH A SHEET OF PAPER TAPED TO THE COVER, WITH THE CORRECT FLT # HAND WRITTEN ON IT. ALL CREW MEMBERS CHKED THE LOG BOOK, BUT FAILED TO VERIFY THE CORRECT LOG WITH ACFT REGISTRATION #. 'WRONG' LOG WAS USED. STRONGER VIGILANCE IS NEEDED BY ALL PERSONNEL IN CONTACT WITH LOG BOOK.
