Narrative:

Our company has failed to keep and maintain complete and proper training records. This has been brought to the attention of the the then chief pilot, who is now director of operations. A request was made for training records re: ground school for differences in training capts one model to the next. The only ground instruction given was in the aircraft, as formal classroom. A questionnaire and 2-PAGE study guide was issued describing the differences between aircraft, and this was to be graded and returned. It was never graded nor returned. After an inquiry and request for the information, the response was simply 'you can fly the aircraft; now do you want it or not?'

Narrative: OUR COMPANY HAS FAILED TO KEEP AND MAINTAIN COMPLETE AND PROPER TRNING RECORDS. THIS HAS BEEN BROUGHT TO THE ATTN OF THE THE THEN CHIEF PLT, WHO IS NOW DIRECTOR OF OPS. A REQUEST WAS MADE FOR TRNING RECORDS RE: GND SCHOOL FOR DIFFERENCES IN TRNING CAPTS ONE MODEL TO THE NEXT. THE ONLY GND INSTRUCTION GIVEN WAS IN THE ACFT, AS FORMAL CLASSROOM. A QUESTIONNAIRE AND 2-PAGE STUDY GUIDE WAS ISSUED DESCRIBING THE DIFFERENCES BTWN ACFT, AND THIS WAS TO BE GRADED AND RETURNED. IT WAS NEVER GRADED NOR RETURNED. AFTER AN INQUIRY AND REQUEST FOR THE INFO, THE RESPONSE WAS SIMPLY 'YOU CAN FLY THE ACFT; NOW DO YOU WANT IT OR NOT?'
