Narrative:

After switching from approach to tower, air carrier X was cleared to land on runway 4 (6005'). Tower controller (trnee) cleared air carrier Y medium large transport for takeoff on runway 15 right (9519'), which intersects with runway 4. At approximately 1 1/2 mi final, tower advised air carrier Y, 'cleared for immediate takeoff or hold short, company on 2...1 1/2 mi final runway 4.' air carrier Y replied, 'rolling.' I observed air carrier Y rounding the corner onto runway 15R. As air carrier Y reached 1500-2000' down runway 15R, tower trnee's supervisor instructed air carrier Y to cancel takeoff clearance. Air carrier Y did not reply and continued to accelerate. No further radio calls were made by the tower. As I reached approximately 1/2 mi final at 200' AGL, I decided that sep would not be adequate at the runway intersection of 4/15R (intersection occurs 1500' beyond the threshold of runway 4). I executed a go around and my aircraft passed approximately 150' above and slightly behind air carrier Y. Air carrier Y had rotated after I began my go around and reached approximately 150' AGL when our flight paths crossed. This incident occurred as a result of the controller trnee's supervisor not taking proper action to ensure that aircraft being handled by the trnee would maintain proper sep during operations conducted on intersecting runways. He did not intervene early enough to cancel air carrier Y's takeoff clearance when it was obvious air carrier Y was slow in beginning its takeoff. When he finally intervened and cancelled air carrier Y's takeoff clearance, he took no further action when air carrier Y failed to reply and continued its takeoff. He did not advise me of air carrier Y as a possible conflict, nor repeat the takeoff cancellation to air carrier Y. After landing, I called the tower and talked with the area supervisor. He had talked to the controllers involved and stated that after the trnee's supervisor cancelled air carrier Y's takeoff clearance, he felt that sep would be adequate (minimal if not legal), and decided no further action was necessary. I disagree! Supplemental information from acn 121116: air carrier Y was cleared for immediate takeoff, runway 15R, bwi, 'traffic 3 mi final intersecting runway.' on runway, while applying power, 'cleared for immediate takeoff or hold position, please advise.' we advised, 'on the roll.' (tower was in training process.) tower later told us that takeoff clearance was cancelled by supervisor which we did not receive. Upon rotation, I heard, 'we're going around,' at which time I noticed an medium large transport at our 2:30 position in a go around mode. We had to decrease our climb to keep a close situation from getting closer.

AT APPROX 1 1/2 MI FINAL, TWR ADVISED ACR Y, 'CLRED FOR IMMEDIATE TKOF OR HOLD SHORT, COMPANY ON 2...1 1/2 MI FINAL RWY 4.' ACR Y REPLIED, 'ROLLING.' I OBSERVED ACR Y ROUNDING THE CORNER ONTO RWY 15R. AS ACR Y REACHED 1500-2000' DOWN RWY 15R, TWR TRNEE'S SUPVR INSTRUCTED ACR Y TO CANCEL TKOF CLRNC. ACR Y DID NOT REPLY AND CONTINUED TO ACCELERATE. NO FURTHER RADIO CALLS WERE MADE BY THE TWR. AS I REACHED APPROX 1/2 MI FINAL AT 200' AGL, I DECIDED THAT SEP WOULD NOT BE ADEQUATE AT THE RWY INTXN OF 4/15R (INTXN OCCURS 1500' BEYOND THE THRESHOLD OF RWY 4).
