Narrative:

I am a flight instructor and was engaged in a check out in the listed helicopter. We were utilizing practice pad number one at long beach airport and were in contact with the local controller at all times. At the listed date and time, we requested takeoff and landing clearance from the local controller. She cleared us for takeoff and landing at pad number one and advised us to remain east of runway 30 at all times. We acknowledged the clearance and proceeded to takeoff in a westerly direction. As we gained translational lift, we started a 180 degree turn to the left to proceed downwind. At no time did we cross or even come within 100' horizontally of runway 30. As we entered the turn I observed an small transport taking off on runway 30. When I first observed him he was still on the ground. I continued to watch it as it broke ground, retracted the landing gear and immediately started a turn to the north. I told my student about the traffic which was now heading directly at us. He observed the traffic about the time I was mentioning it to him and he started a descent and stopped the turn so that we could pass behind small transport X. Small transport X passed over us with what I would estimate was 100' of clearance. I asked the controller over the radio what the small transport was doing and she told me that she didn't know, that she had cleared it for a straight out departure. A voice that I presume to be the pilot of the small transport then came on the radio and said that he was forced to maneuver to avoid a collision with the helicopter that had flown over the runway. The controller told small transport X that the helicopter had never flown over the runway and he should have continued straight out. I told him that he had turned into us and instead of avoiding a collision situation had actually caused it. There was no further conversation about the incident.

Title: A DEPARTING SMT CAME IN CLOSE PROX TO A HELICOPTER.

Narrative: I AM A FLT INSTRUCTOR AND WAS ENGAGED IN A CHECK OUT IN THE LISTED HELI. WE WERE UTILIZING PRACTICE PAD NUMBER ONE AT LONG BEACH ARPT AND WERE IN CONTACT WITH THE LCL CTLR AT ALL TIMES. AT THE LISTED DATE AND TIME, WE REQUESTED TKOF AND LNDG CLRNC FROM THE LCL CTLR. SHE CLRED US FOR TKOF AND LNDG AT PAD NUMBER ONE AND ADVISED US TO REMAIN E OF RWY 30 AT ALL TIMES. WE ACKNOWLEDGED THE CLRNC AND PROCEEDED TO TKOF IN A WESTERLY DIRECTION. AS WE GAINED TRANSLATIONAL LIFT, WE STARTED A 180 DEG TURN TO THE LEFT TO PROCEED DOWNWIND. AT NO TIME DID WE CROSS OR EVEN COME WITHIN 100' HORIZONTALLY OF RWY 30. AS WE ENTERED THE TURN I OBSERVED AN SMT TAKING OFF ON RWY 30. WHEN I FIRST OBSERVED HIM HE WAS STILL ON THE GND. I CONTINUED TO WATCH IT AS IT BROKE GND, RETRACTED THE LNDG GEAR AND IMMEDIATELY STARTED A TURN TO THE N. I TOLD MY STUDENT ABOUT THE TFC WHICH WAS NOW HDG DIRECTLY AT US. HE OBSERVED THE TFC ABOUT THE TIME I WAS MENTIONING IT TO HIM AND HE STARTED A DSCNT AND STOPPED THE TURN SO THAT WE COULD PASS BEHIND SMT X. SMT X PASSED OVER US WITH WHAT I WOULD ESTIMATE WAS 100' OF CLRNC. I ASKED THE CTLR OVER THE RADIO WHAT THE SMT WAS DOING AND SHE TOLD ME THAT SHE DIDN'T KNOW, THAT SHE HAD CLRED IT FOR A STRAIGHT OUT DEP. A VOICE THAT I PRESUME TO BE THE PLT OF THE SMT THEN CAME ON THE RADIO AND SAID THAT HE WAS FORCED TO MANEUVER TO AVOID A COLLISION WITH THE HELI THAT HAD FLOWN OVER THE RWY. THE CTLR TOLD SMT X THAT THE HELI HAD NEVER FLOWN OVER THE RWY AND HE SHOULD HAVE CONTINUED STRAIGHT OUT. I TOLD HIM THAT HE HAD TURNED INTO US AND INSTEAD OF AVOIDING A COLLISION SITUATION HAD ACTUALLY CAUSED IT. THERE WAS NO FURTHER CONVERSATION ABOUT THE INCIDENT.
