Narrative:

After takeoff; the gear door open light remained illuminated. We accomplished the gear door open light in cruise QRH procedure. We advised ATC of reduced speed limitation and contacted dispatch. Dispatch sent new time and fuel log for reduced airspeed. We noted no secondary indications of vibration or extra fuel burn. Upon extending the gear on final approach; the gear door open light remained illuminated. We accomplished the go-around; declared an emergency and accomplished the gear door open light with left; nose; and right gear lights green QRH procedure. We received vectors and contacted dispatch and maintenance. Requested and received emergency equipment and a long runway. Flew the ILS and stopped on the runway. Emergency personnel and maintenance inspected the aircraft. Maintenance secured the gear doors and pinned the gear. We taxied to the gate followed by maintenance and emergency vehicles.
