Narrative:
I was training a fairly new developmental when aircraft X came in wanting to execute practice approaches at fmh. Upon executing his first approach; he came back to us as expected due to multiple approach requests. Upon his climb out; aircraft X advised us that they could not reach the tower. [He] also told us that he had the old tower frequency of 121.0; which was changed approximately three months ago to 128.425. I advised aircraft X of the new frequency and immediately called the tower to advise them of this. It appears that the frequency change has not been updated on the approach plates. I believe that the pilot should have received the appropriate briefing. From the controller perspective; we should give the tower frequency to every aircraft to avoid any confusion; especially at airports that have had a recent frequency change.
