Narrative:

Departed fmy for lan on an air ambulance flight. Final altitude filed for was FL430. We were cleared to climb to FL430 and given a vector for direct lan. Climbing through FL420 for FL430 the left engine flamed out. We requested a lower altitude of FL240. The controller gave us an altitude of FL350 and said if we needed lower he would give us a vector. We accepted FL350 on course, believing we were clear of traffic. As we leveled off at FL350 the airspeed continued to bleed off. The captain instructed me to request a lower altitude. Due to congestion on the frequency it was approximately 30 seconds before I could make contact with a controller. I requested lower, but the controller said to maintain FL350. I then told the controller we had to descend and we would take a vector. By this point we had slipped 400' below our assigned altitude. The controller instructed us to squawk 7700 and gave us a vector for traffic avoidance. We continued our descent to a lower altitude and completed a successful restart. We accepted FL350 on course because we believed that we would be cleared to a lower altitude before reaching FL350. Supplemental information from acn 97141: center offered vector or on course. Requested on course. Engine successfully restated at 19000'. Center issued clearance to climb and continue on course.

CLBING THROUGH FL420 FOR FL430 THE LEFT ENG FLAMED OUT. WE REQUESTED A LOWER ALT OF FL240. THE CTLR GAVE US AN ALT OF FL350 AND SAID IF WE NEEDED LOWER HE WOULD GIVE US A VECTOR. WE ACCEPTED FL350 ON COURSE, BELIEVING WE WERE CLR OF TFC. AS WE LEVELED OFF AT FL350 THE AIRSPD CONTINUED TO BLEED OFF. THE CAPT INSTRUCTED ME TO REQUEST A LOWER ALT.
