Narrative:

On post flight walkaround a rep of the FAA san antonio branch did a routine ramp check of our operation. After checking and recording our credentials he departed. Discussing the ramp check with the captain I realized I inadvertently omitted traffic violations on my last medical check. They consisted of 2 speeding tickets in PA 4 yrs ago, 1 speeding ticket in PA in 5/87, and 2 speeding tickets in nc in the past 15 months. I immediately forwarded these violations to the aeronautical medical certification branch, aam-130 civil aeronautical medical institute, P.O. Box 25082, oklahoma city, ok, 73125 in 10/88.

Title: TRAFFIC VIOLATIONS NOT REPORTED ON MEDICAL FORM.
