Narrative:

Supplemental information from acn 96046: the preflight was completed with all engine cowlings appearing closed and locked. However, a crack was noticed in the #4 engine tail cone. At that point maintenance began repairs to the #4 tail cone. I periodically checked the progress of the repair and at no time did I notice the cowling on the #4 engine opened. Supplemental information from acn 96112: later we discovered that the cowl had become dislodged at ewr airport on takeoff. At ewr maintenance crews were replacing a patch on engine #4 exhaust cone. It was the first cold day of the season. It was rainy accompanied with a 14-15 KT wind at the time the repair was in progress.

Title: ACR WDB LOST A PIECE OF ENGINE COWL ON TKOF FROM EWR. PROBLEM WAS NOT FOUND UNTIL ACFT LANDED AT LGW.

Narrative: SUPPLEMENTAL INFO FROM ACN 96046: THE PREFLT WAS COMPLETED WITH ALL ENG COWLINGS APPEARING CLOSED AND LOCKED. HOWEVER, A CRACK WAS NOTICED IN THE #4 ENG TAIL CONE. AT THAT POINT MAINT BEGAN REPAIRS TO THE #4 TAIL CONE. I PERIODICALLY CHKED THE PROGRESS OF THE REPAIR AND AT NO TIME DID I NOTICE THE COWLING ON THE #4 ENG OPENED. SUPPLEMENTAL INFO FROM ACN 96112: LATER WE DISCOVERED THAT THE COWL HAD BECOME DISLODGED AT EWR ARPT ON TKOF. AT EWR MAINT CREWS WERE REPLACING A PATCH ON ENG #4 EXHAUST CONE. IT WAS THE FIRST COLD DAY OF THE SEASON. IT WAS RAINY ACCOMPANIED WITH A 14-15 KT WIND AT THE TIME THE REPAIR WAS IN PROGRESS.
