Narrative:

I was not watching the final [approach] as I should have been and loaded the runway in front of an aircraft on 3 mile final. This caused a go-around and I was so upset that I had not seen the aircraft on final that I didn't issue go-around instructions that would have negated the need for visual separation. Additionally; I didn't get the required information from the pilot to apply visual separation. Recommendation; stand up while working local and pay attention.

Title: Tower Controller described a go-around event resulting from issuing a takeoff clearance without first observing landing traffic; adding proper visual separation phraseology was not used during the occurrence.
