Narrative:
First class physical lapsed just after upgrade from first officer to captain. Date of physical 7/xx/87. Training was finished in 12/87 and 1/88 was month. Company records did not update for notification and month of 2/88 was first with second class physical. I was alerted on 3/tue/88 and a new physical was secured. This report is being mailed from memphis on 3/tue/88. Callback conversation with reporter revealed the following: original report reference captain flying with class ii physical vs a class I was sent out same day reporter was notified his physical was out of date. The report never reached ASRS office, but followup phone call was made by reporter approximately 2 weeks later. After FAA notification of proposed action, reporter made subsequent phone calls re: the report which was never received. At ASRS request, a copy of the original (this report) was sent. Reporter was given 2 weeks off west/O pay by his company and further FAA action is pending.

Narrative: FIRST CLASS PHYSICAL LAPSED JUST AFTER UPGRADE FROM F/O TO CAPT. DATE OF PHYSICAL 7/XX/87. TRNING WAS FINISHED IN 12/87 AND 1/88 WAS MONTH. COMPANY RECORDS DID NOT UPDATE FOR NOTIFICATION AND MONTH OF 2/88 WAS FIRST WITH SECOND CLASS PHYSICAL. I WAS ALERTED ON 3/TUE/88 AND A NEW PHYSICAL WAS SECURED. THIS RPT IS BEING MAILED FROM MEMPHIS ON 3/TUE/88. CALLBACK CONVERSATION WITH REPORTER REVEALED THE FOLLOWING: ORIGINAL RPT REF CAPT FLYING WITH CLASS II PHYSICAL VS A CLASS I WAS SENT OUT SAME DAY RPTR WAS NOTIFIED HIS PHYSICAL WAS OUT OF DATE. THE RPT NEVER REACHED ASRS OFFICE, BUT FOLLOWUP PHONE CALL WAS MADE BY RPTR APPROX 2 WKS LATER. AFTER FAA NOTIFICATION OF PROPOSED ACTION, RPTR MADE SUBSEQUENT PHONE CALLS RE: THE RPT WHICH WAS NEVER RECEIVED. AT ASRS REQUEST, A COPY OF THE ORIGINAL (THIS RPT) WAS SENT. RPTR WAS GIVEN 2 WKS OFF W/O PAY BY HIS COMPANY AND FURTHER FAA ACTION IS PENDING.
