Narrative:

Condition night VMC about 5 to 10 NM northeast of cedes intersection on a heading of roughly 300 degrees; descending from 12;000 to 11;000 ft. The flight crew experienced a green laser illumination sighted at the aircraft for approximately 3 to 4 seconds. The laser broadcast four or five sweeps with two direct sightings in the cockpit. The captain received direct exposure to the left eye. The event was reported to nct approach as a PIREP. Also; the laser illumination was reported to the company after landing. The source of the laser appeared to be ground based from the vicinity of the northern san jose or milpitas area at a distance of 5 SM due north of the san jose international airport. After noting the area and street locations; a local road map revealed the source area to be berryessa creek park. The aircraft continued the approach and landing without further incident.
