Narrative:

On departure at approximately 500 ft MSL we received an intermittent level one 'eng 3 vib high' alert. No vibration was felt by crew and all other engine parameters indicated normal. Alert continued to go on and off and fluctuate between .5 and 4.0 until reaching approximately 10;000 ft. Alert extinguished for remainder of flight and flight progressed normally. Upon post-flight inspection it was evident that substantial damage had occurred to the number 3 engine fan blades and inner cowling. Crew made a logbook entry and proceeded to hotel.
