Narrative:

It was called to my attention that the survival strap installations on the cabin door 2-LH and 2-RH slide installations were not done properly. This was a result of a inbound logbook item on the door 1-LH. All of the slide installation on the aircraft were done wrong; all four slides. I had checked these on an 'a' check; the items were inspected and flagged as service difficulty report (sdr) item. Why were they now incorrectly done? This is an obvious issue and needs to be looked in to.
