Narrative:

The crew was in the process of weighing the aircraft. We took the aircraft off the scales; reset the scales; and pushed the plane back on the scales for the second weigh. The tug driver called to set the brakes. The brakes were set as indicated by the brake operator. The man on the tow bar asked if the brakes were set. Since I could see both the tug driver and the brake operator; I indicated the brakes were set. I was standing by the tow bar at the tug side; on the right hand side; looking at the aircraft. He released the tow bar; and as soon as he released the tow bar; the aircraft began to roll backwards. The next thing that happened; the emergency slide on the service door fell to the ground and everyone started screaming. Plane rolling. A chalk was thrown under the wheel (left side). The plane rolled over it and did not stop or slow down. Plane stopped when it hit the hangar door.
