Narrative:

Flight crew noticed #1 engine reverser cowl eroded at approximately 6 o'clock position. Determined area of erosion within structural repair manual (srm) allowable limits at time of deferral. Cleaned area; taped area and stated in text item; to be addressed next 'a' check. Item not addressed until july 2009; and area of erosion much greater than area at time deferral created. Area had been retaped many times; prior to me being notified by my supervisor that an investigation was being conducted. Due to item not being addressed as indicated in initial deferral; area of damage greatly increased.
