Narrative:

We received taxi out instructions in phx to; 'taxi via D; D13; hold short of east taxiway.' there were several aircraft holding short of runway 25R on east; and one aircraft on right holding short of D. The airport diagram has D13 as the last taxiway that you can make a 90 degree right turn onto east. We taxied to the end of D and made the 90 degree right turn; cutting off the aircraft that was holding on right. Tower at that point told us that we screwed up and that we were on right taxiway. In order to get us out of the way tower then gave us clearance to taxi down runway 25R and exit at F12; taxi on F; hold short of runway 25R. We obviously missed D13. Unfortunately; I don't recall what all the taxiway signage was when we were taxiing on D; past D13. What is there and what is on the airport diagram don't appear to be the same but not sure.

Title: B737-700 flight crew taxiing out for departure at PHX blocked the entrance to Runway 25R when they turned onto the wrong taxiway.

Narrative: We received taxi out instructions in PHX to; 'Taxi via D; D13; hold short of E Taxiway.' There were several aircraft holding short of Runway 25R on E; and one aircraft on R holding short of D.
