Narrative:

Following winter storm on 2/tue/88 had aircraft deiced by XXX service, brainard field, hartford, ct, 2/sat/88. Departed brainard en route islip mac arthur field at XX00 local. Approximately 15 mins after departure while in cavu conditions entire windshield iced over reducing forward visibility significantly. Contacted ny approach, informed them of situation and requested ILS runway 24 rather than ATIS reported visual to runway 28. Subsequently landed west/O incident. Called XXXX to report incident. Maintenance supervisor speculated improper alcohol/water mixture had been used and apologized. Note: non instrument rated pilot would have had great difficulty navigating and executing approach/landing.
