Narrative:

Preflight had been completed as per aircraft manual and the aircraft was being refueled. The fueler came to the aircraft and filled the aircraft with the amount of fuel requested and departed. I got into the aircraft and began the flight. We taxied out and departed as normal. There were no problems with the aircraft during the flight and we returned as normal. After we returned we found a fuel cap on taxiway B and one at the departure end of the runway.

Title: A DECATHLON PILOT TOOK OFF WITHOUT HIS FUEL CAPS PROPERLY SECURED AFTER REFUELING.

Narrative: PREFLT HAD BEEN COMPLETED AS PER ACFT MANUAL AND THE ACFT WAS BEING REFUELED. THE FUELER CAME TO THE ACFT AND FILLED THE ACFT WITH THE AMOUNT OF FUEL REQUESTED AND DEPARTED. I GOT INTO THE ACFT AND BEGAN THE FLT. WE TAXIED OUT AND DEPARTED AS NORMAL. THERE WERE NO PROBS WITH THE ACFT DURING THE FLT AND WE RETURNED AS NORMAL. AFTER WE RETURNED WE FOUND A FUEL CAP ON TXWY B AND ONE AT THE DEP END OF THE RWY.
