Narrative:

Taking off from teb we were given the teb 5 departure; and climbed to 2000 ft before crossing 4.5 DME. We both read the departure prior to departing runway 24. I read out loud fly runway heading to 1500 ft then right turn to 280 degrees and up to 2000 ft. There is an altitude box that states maintain 1500 ft till passing 4.5 DME off teb that was overlooked.
