Narrative:

Aircraft X; a P32R/G; filed IFR pit..bsv..halie..gyy. Issued PIT8 departure. Aircraft was cleared for takeoff on runway 28R and issued runway heading. Aircraft checked in with departure radar north 'leveling at 1700 MSL.' there are many towers and other obstructions in the area at that altitude. Pilot must have misread PIT8 SID/departure and leveled at 1700 ft MSL. Issued an immediate climb to 4000 ft MSL as per the PIT8 SID/departure. Changes have been made to the PIT8 textually for the next issue of charts. It will be PIT9 and will take effect on jun/xa/08. This is a very dangerous problem. It is not just one pilot! Many pilots have done the exact same thing. Being a pilot myself; I don't see how they are confusing the altitudes. But; there are way too many events for it to be an isolated problem. I feel the problem has come from combining departures and SID's rather than training pilots on the difference. I hope this newest revision stops the problem.

Narrative: ACFT X; A P32R/G; FILED IFR PIT..BSV..HALIE..GYY. ISSUED PIT8 DEP. ACFT WAS CLRED FOR TKOF ON RWY 28R AND ISSUED RWY HDG. ACFT CHKED IN WITH DEP RADAR N 'LEVELING AT 1700 MSL.' THERE ARE MANY TWRS AND OTHER OBSTRUCTIONS IN THE AREA AT THAT ALT. PLT MUST HAVE MISREAD PIT8 SID/DEP AND LEVELED AT 1700 FT MSL. ISSUED AN IMMEDIATE CLB TO 4000 FT MSL AS PER THE PIT8 SID/DEP. CHANGES HAVE BEEN MADE TO THE PIT8 TEXTUALLY FOR THE NEXT ISSUE OF CHARTS. IT WILL BE PIT9 AND WILL TAKE EFFECT ON JUN/XA/08. THIS IS A VERY DANGEROUS PROB. IT IS NOT JUST ONE PLT! MANY PLTS HAVE DONE THE EXACT SAME THING. BEING A PLT MYSELF; I DON'T SEE HOW THEY ARE CONFUSING THE ALTS. BUT; THERE ARE WAY TOO MANY EVENTS FOR IT TO BE AN ISOLATED PROB. I FEEL THE PROB HAS COME FROM COMBINING DEPS AND SID'S RATHER THAN TRAINING PLTS ON THE DIFFERENCE. I HOPE THIS NEWEST REVISION STOPS THE PROB.
