Narrative:

When PF called for flaps up on initial climb; a le slat disagree EICAS annunciated. PF took over radios and asked for 6000 ft and vectors to remain in the local area for possible return to ZZZ. PNF accomplished QRH procedure with no help (ie EICAS message remained illuminated) which drives the crew to accomplish le asym irregular procedure. PNF accomplished this procedure; then PF gave first officer the controls while still on vectors at 6000 ft. Captain contacted dispatch via phone patch; and dispatch brought maintenance on to confer. No help. Dispatch and captain both agreed to a return to ZZZ; declared an emergency and requested emergency vehicles. Controls were changed back to the captain for landing in visual conditions. Crew was able to run te flaps out to position 20 per FM procedure and aircraft landed uneventfully. Entered maintenance log item. This aircraft has chronic history with leading edge slats with 4 previous write ups. Hope the aircraft is repaired prior to the next flight. This irregular drives the crew to return to the departure field if le slats remain out due to a 230 KTS speed restriction; a 20000 ft altitude restriction; and unreliable FMC fuel predictions with flaps or slats extended.
