Narrative:

Because business class needed 4 of the first class meals; I was putting those together (3 items each in 4 different meal tins) during a long taxi. We were understaffed and wanted the service to be ready to go soon after takeoff. We never heard a 'prepare for takeoff' announcement from the cockpit (discovered later that cockpit microphone was inoperative) so I thought I had time to finish this task. I was still standing when we suddenly took off.

Title: FA WAS READYING MEALS IN THE GALLEY DURING TAXI. THE 'PREPARE FOR TAKEOFF' ANNOUNCEMENT WAS NOT HEARD; AND THE B767-300 TOOK OFF WITH THE FLT ATTENDANT STILL STANDING.

Narrative: BECAUSE BUSINESS CLASS NEEDED 4 OF THE FIRST CLASS MEALS; I WAS PUTTING THOSE TOGETHER (3 ITEMS EACH IN 4 DIFFERENT MEAL TINS) DURING A LONG TAXI. WE WERE UNDERSTAFFED AND WANTED THE SVC TO BE READY TO GO SOON AFTER TKOF. WE NEVER HEARD A 'PREPARE FOR TKOF' ANNOUNCEMENT FROM THE COCKPIT (DISCOVERED LATER THAT COCKPIT MIKE WAS INOP) SO I THOUGHT I HAD TIME TO FINISH THIS TASK. I WAS STILL STANDING WHEN WE SUDDENLY TOOK OFF.
