Narrative:
Jet departures off charlotte on hornet 3 are coming off airport on the incorrect frequency. Specifically; the departures that are on the hornet 3 (SID); meril transition; come to the west departure frequency (an operational error occurred at clt in the past year because of a similar situation). On the SID; it states departure frequency for jotta; naley; xaver; debie; haray is 120.5; and an additional note says meril departure frequency is 124.0. Many pilots don't read that far; and have been given the hornet 2 departure for so many yrs; just assume that the old frequency is the same for all hornet 3 departures. I report this to the supervisor; they don't log it or follow up on it; to my knowledge.
