Narrative:

On climb out; requested deviation for WX. Was told unable due to traffic. ATC notified us that he showed us deviating; and we responded that we were still flying as filed. He then said we were filed molen direct boxer. We responded we were cleared as filed; but direct boxer would work fine. Controller said nothing further but; after leveloff; we reviewed the pre departure clearance and discovered that our revised segment showed 'MOLEN3 molen boxer.' all 3 of us had misread the pre departure clearance; and confirmed (incorrectly) that the pre departure clearance showed 'MOLEN3; as filed.' simply misread the pre departure clearance. In retrospect; I would have benefited from reading the pre departure clearance verbatim; and confirming the clearance in the FMC with both first officer's confirming just as we would on international flight plans.

Title: B747-400 FLT CREW FAILS TO NOTE DEP CLRNC WAS MOLEN SID TO MOLEN INTERSECTION; THENCE DIRECT TO BOXER.

Narrative: ON CLBOUT; REQUESTED DEV FOR WX. WAS TOLD UNABLE DUE TO TFC. ATC NOTIFIED US THAT HE SHOWED US DEVIATING; AND WE RESPONDED THAT WE WERE STILL FLYING AS FILED. HE THEN SAID WE WERE FILED MOLEN DIRECT BOXER. WE RESPONDED WE WERE CLRED AS FILED; BUT DIRECT BOXER WOULD WORK FINE. CTLR SAID NOTHING FURTHER BUT; AFTER LEVELOFF; WE REVIEWED THE PDC AND DISCOVERED THAT OUR REVISED SEGMENT SHOWED 'MOLEN3 MOLEN BOXER.' ALL 3 OF US HAD MISREAD THE PDC; AND CONFIRMED (INCORRECTLY) THAT THE PDC SHOWED 'MOLEN3; AS FILED.' SIMPLY MISREAD THE PDC. IN RETROSPECT; I WOULD HAVE BENEFITED FROM READING THE PDC VERBATIM; AND CONFIRMING THE CLRNC IN THE FMC WITH BOTH FO'S CONFIRMING JUST AS WE WOULD ON INTL FLT PLANS.
