Narrative:

Several items: possible violation of far 121.483 if we depart as scheduled due to 3-MAN crew scheduled over 12 hours in 24. Second: extremely sick passenger nearly caused an in-flight divert. Dispatch and physician on call utilized but was inaudible. Paramedics were requested through dispatch and VHF. A wheelchair showed up. Third: the #4 flight attendant reported an injury on duty after falling over a child in business class and injuring her back. I don't know if she is able to perform emergency duties on the return flight. Fourth: purser informed me that her contract was violated by my not delaying to board flight attendant meals due to flight being over 12 hours. She also questioned the safety of departing as scheduled.

Narrative: SEVERAL ITEMS: POSSIBLE VIOLATION OF FAR 121.483 IF WE DEPART AS SCHEDULED DUE TO 3-MAN CREW SCHEDULED OVER 12 HRS IN 24. SECOND: EXTREMELY SICK PAX NEARLY CAUSED AN INFLT DIVERT. DISPATCH AND PHYSICIAN ON CALL UTILIZED BUT WAS INAUDIBLE. PARAMEDICS WERE REQUESTED THROUGH DISPATCH AND VHF. A WHEELCHAIR SHOWED UP. THIRD: THE #4 FLT ATTENDANT RPTED AN INJURY ON DUTY AFTER FALLING OVER A CHILD IN BUSINESS CLASS AND INJURING HER BACK. I DON'T KNOW IF SHE IS ABLE TO PERFORM EMER DUTIES ON THE RETURN FLT. FOURTH: PURSER INFORMED ME THAT HER CONTRACT WAS VIOLATED BY MY NOT DELAYING TO BOARD FLT ATTENDANT MEALS DUE TO FLT BEING OVER 12 HRS. SHE ALSO QUESTIONED THE SAFETY OF DEPARTING AS SCHEDULED.
