Narrative:

Upon lowering the landing gear within the pilot's information manual vlo (maximum landing gear operating speed); the main gear indicated down and locked; the nose gear indicated unsafe; the gear-unlocked light remained illuminated; and the hydraulic power pack was audible and continued to run with its associated annunciator. After manual extension per the pilot's information manual; the nose gear again indicated unsafe; and the red gear-unlocked light was illuminated. Maneuvering to load the aircraft failed to result in a safe indication. After a fly-by; the tower said that the gear seemed to be down; however; the nose gear unsafe indication remained with an associated red gear-unlocked light. A practice approach to 100 ft AGL was conducted followed by an actual approach in which the engine was shut down proximate to main wheel touchdown. As the nose was lowered; the nosewheel collapsed resulting in damage to the propeller and lower cowling.

Title: TB21 PILOT HAS UNSAFE NOSE GEAR INDICATION AND AFTER UNSUCCESSFUL ATTEMPT TO LOWER MANUALLY; LANDING IS MADE WITH SUBSEQUENT NOSE GEAR COLLAPSE.

Narrative: UPON LOWERING THE LNDG GEAR WITHIN THE PLT'S INFO MANUAL VLO (MAX LNDG GEAR OPERATING SPD); THE MAIN GEAR INDICATED DOWN AND LOCKED; THE NOSE GEAR INDICATED UNSAFE; THE GEAR-UNLOCKED LIGHT REMAINED ILLUMINATED; AND THE HYD PWR PACK WAS AUDIBLE AND CONTINUED TO RUN WITH ITS ASSOCIATED ANNUNCIATOR. AFTER MANUAL EXTENSION PER THE PLT'S INFO MANUAL; THE NOSE GEAR AGAIN INDICATED UNSAFE; AND THE RED GEAR-UNLOCKED LIGHT WAS ILLUMINATED. MANEUVERING TO LOAD THE ACFT FAILED TO RESULT IN A SAFE INDICATION. AFTER A FLY-BY; THE TWR SAID THAT THE GEAR SEEMED TO BE DOWN; HOWEVER; THE NOSE GEAR UNSAFE INDICATION REMAINED WITH AN ASSOCIATED RED GEAR-UNLOCKED LIGHT. A PRACTICE APCH TO 100 FT AGL WAS CONDUCTED FOLLOWED BY AN ACTUAL APCH IN WHICH THE ENG WAS SHUT DOWN PROXIMATE TO MAIN WHEEL TOUCHDOWN. AS THE NOSE WAS LOWERED; THE NOSEWHEEL COLLAPSED RESULTING IN DAMAGE TO THE PROP AND LOWER COWLING.
