Narrative:

Descending on the GS on the visual with gear and final descent checklist complete; we selected flaps 25 degrees and got 2 EICAS messages: 'trailing edge flap disagreement' and 'leading edge slat disagreement.' we elected to go around and the tower gave us an altitude and heading. At 3000 ft we completed the appropriate irregular checklists; declared an emergency; and asked for equipment to stand by. We sent an ACARS message to dispatch. We landed in accordance with flaps 20 degrees reference plus wind correction with no difficulties and taxied to the gate.
