B737 was given a vector prior to frequency change. The vector carried B737 into an adjacent controller's airspace without a pointout and loss of separation occurred with an aircraft departing sjc.

Title: NCT CTLR EXPERIENCED OPERROR AT 4000 WHEN FAILING TO COORD ACFT HEADING WITH RECEIVING CTLR; RESULTING IN AN AIRSPACE/ACFT CONFLICTION.
Narrative: B737 WAS GIVEN A VECTOR PRIOR TO FREQ CHANGE. THE VECTOR CARRIED B737 INTO AN ADJACENT CTLR'S AIRSPACE WITHOUT A POINTOUT AND LOSS OF SEPARATION OCCURRED WITH AN ACFT DEPARTING SJC.
