Narrative:

Cockpit established communication with ground crew. Cleared for engine starts. During 2ND engine start ground crew walked away. No communication with ramp following after start flow. Copilot and I checked for all clear on both sides of aircraft. 3 ground crew were off to the left of airplane and gave a 'thumbs up' to me. I started taxi out after ground clearance; but 1 ramper was still hooked in to aircraft with headset. Copilot and I heard nothing and saw nothing. Luckily ramper unhooked and made it clear of aircraft and no one was hurt. Have ramper stand in front of aircraft with wands visible to both pilots. Ensure a positive wave-off; and confirm all flight to ground and ground to flight communications.

Title: A B737-400 FLT CREW WAS DISPATCHED FROM THE GATE WITH THE RAMP CREW USING NONSTANDARD AND UNSAFE PROCEDURES.

Narrative: COCKPIT ESTABLISHED COM WITH GND CREW. CLRED FOR ENG STARTS. DURING 2ND ENG START GND CREW WALKED AWAY. NO COM WITH RAMP FOLLOWING AFTER START FLOW. COPLT AND I CHKED FOR ALL CLR ON BOTH SIDES OF ACFT. 3 GND CREW WERE OFF TO THE L OF AIRPLANE AND GAVE A 'THUMBS UP' TO ME. I STARTED TAXI OUT AFTER GND CLRNC; BUT 1 RAMPER WAS STILL HOOKED IN TO ACFT WITH HEADSET. COPLT AND I HEARD NOTHING AND SAW NOTHING. LUCKILY RAMPER UNHOOKED AND MADE IT CLR OF ACFT AND NO ONE WAS HURT. HAVE RAMPER STAND IN FRONT OF ACFT WITH WANDS VISIBLE TO BOTH PLTS. ENSURE A POSITIVE WAVE-OFF; AND CONFIRM ALL FLT TO GND AND GND TO FLT COMS.
