Narrative:

Background: T/M/south sabreliner T39G is a military owned aircraft maintained by contractor. By contract only FAA-pma parts are to be utilized. Incident: prior to takeoff the pilot experienced an uncommanded right engine shutdown; accompanied by a fuel leak (right engine). Maintenance reported the fuel flow transmitter to be the cause of both the fuel leak and uncommanded engine shutdown. Cause: the fuel flow had failed internally due to (6 each) allen head bolts backing out. The F/F transmitter separated into two halves when it was removed from the engine. When all 6 of the internal bolt backed out it broke an internal guide tube. Preventive action: safety wire allen head bolts.

Title: A MIL T39G SABRELINER ACFT HAD AN UNCOMMANDED R ENGINE SHUTDOWN ACCOMPANIED BY A FUEL LEAK DUE TO FAILURE OF FUEL FLOW XMITTER.

Narrative: BACKGROUND: T/M/S SABRELINER T39G IS A MIL OWNED ACFT MAINTAINED BY CONTRACTOR. BY CONTRACT ONLY FAA-PMA PARTS ARE TO BE UTILIZED. INCIDENT: PRIOR TO TKOF THE PLT EXPERIENCED AN UNCOMMANDED R ENG SHUTDOWN; ACCOMPANIED BY A FUEL LEAK (R ENG). MAINT RPTED THE FUEL FLOW XMITTER TO BE THE CAUSE OF BOTH THE FUEL LEAK AND UNCOMMANDED ENG SHUTDOWN. CAUSE: THE FUEL FLOW HAD FAILED INTERNALLY DUE TO (6 EACH) ALLEN HEAD BOLTS BACKING OUT. THE F/F XMITTER SEPARATED INTO TWO HALVES WHEN IT WAS REMOVED FROM THE ENG. WHEN ALL 6 OF THE INTERNAL BOLT BACKED OUT IT BROKE AN INTERNAL GUIDE TUBE. PREVENTIVE ACTION: SAFETY WIRE ALLEN HEAD BOLTS.
